Health Maintenance

Synergy provides in-home health care visits monthly as a baseline.  We are proactive in our healthcare practice, working to catch health issues early for best medical outcomes. By being proactive, the amount of treatment needed is less and we can prevent ER visits and hospitalizations. We eliminate the need for our patients to travel outside of their homes by coming to them. We provide in-house and telemedicine to assure our patients continue to have optimal quality of life. 

Our providers are highly experienced in: 

• Hypertension, Congestive Heart Failure (CHF), Atrial Fibrillation (Afib) with anticoagulation monitoring
• Chronic Obstructive Pulmonary Disease (COPD)
• Diabetes
• Alzheimer's Disease and Dementia, including agitation, aggression, and violent behaviors
• Mood disorders, including depression and bipolar, Anxiety Disorder, Personality Disorder, Psychotic Disorders, and PTSD
• Wound management, including pressure ulcers (bed sores), diabetic ulcers, stasis ulcers, and cryotherapy

Follow Up Assessment

When making any changes to medications or our patient’s plan of care, we follow up to ensure the treatment was effective. If not effective, we choose another option until our patients find relief. 

Acute Care Visits - Telemedicine

Telemedicine allows us to perform an physical assessment without delay in care anytime of day. Acute care visits prevent delay in care, which can ultimately lead to an emergency room visit and hospitalization.  Gone are the days of waiting weeks to see "the doctor". Our patients are seen "same day" or within 24 hours. Telemedicine allows us to see our patients prior to be sent the the ER as we are able to treat a large percentage of patients in-home. There is no time of day where our providers are unavailable. We provide visits as 2am just as easily as we do at 2pm and with the same level of care and attention.

On-Site Diagnostics

We collaborate with imaging and outside lab services, bringing them to our patients. With our partnerships, we can provide in-home lab services to obtain routine or urgent bloodwork or other types of specimens. Imaging services come in-house to perform X-Ray, EKG, and Doppler services, again preventing delay in care and the additional stress of spending hours in the ER.

Chronic Care Management (CCM)

Unfortunately, we all age and in the process, develop long-term medical diseases/disorders. Medicare encourages providers to monitor these medical conditions by the ongoing gathering of information from families and caregivers. This allows for identifying trends and immediately implementing treatments, reducing amount of treatment needed and preventing ER visits and hospitalizations. 

Remote Patient Monitoring (RPM)

With the assistance of family and caregivers, we can monitor patients on a daily basis. We provide our patients with unstable high blood pressure monitoring equipment, giving us the ability to monitor blood pressures on a daily basis. This information allows Synergy Medical Services to monitor trends and prevent hypertensive episodes before it requires a visit to the ER and hospitalization.
